How long does cell immunotherapy need to be continued for to be considered as approaching the goal of cure?

Mr. Zhang is one of the patients who have been following NKT cell therapy for a long time, now for over 6 years. Having spent so much time together, he has become an old friend of our team. Over these 6 years, we have witnessed Mr. Zhang's transformation from anxiety over repeated hospitalizations to a relaxed and contented life at home with his grandchildren, solely due to NKT cell immunotherapy. We are deeply gratified and proud of his progress.
Such cases are of great research value. Now, what is his current status?

Mr. Zhang is a patient with suspected primary colon cancer and pancreatic cancer. Before receiving NKT cell immunotherapy, he continuously developed new metastatic lesions, making his medical history particularly complex.

In February 2012, he underwent "left hemicolectomy" for cancer in the splenic flexure of the colon: moderately differentiated adenocarcinoma of the transverse colon, with cancer tissue invading the serosal layer, peritoneal fibrous fat showing cancer tissue infiltration, vascular thrombosis visible, lymph nodes (1/13), pT4aN1M0.

After surgery, he underwent two rounds of intraperitoneal hyperthermic perfusion chemotherapy, six cycles of Folfox chemotherapy, and two courses of single-agent Xeloda oral administration. Regular follow-up examinations were then conducted.

However, two years later (February 2014), a suspicious primary tumor was found in the tail of the pancreas, and he underwent surgery and chemotherapy again; but one year later (April 2015), multiple enlarged lymph nodes were found in the hepatic hilum and retroperitoneum, and he underwent two more cycles of chemotherapy. Multiple surgeries and chemotherapy have left Mr. Zhang with lingering fears and an inability to reduce the risk of recurrence and metastasis.

In July 2015, Mr. Zhang accepted our recommendation for NKT cell therapy, following a regimen of one course per month. So far, he has maintained this for 70 months without any signs of recurrence or metastasis. He reports that his hair has turned from white to black, his skin has become delicate and smooth, his diet and sleep quality are excellent, his mental state is very good, and he feels happy and content.

Looking back on these six years of NKT treatment (without any other therapeutic interventions), we can observe an interesting phenomenon:

In the first year of treatment, the tumor was still active—retroperitoneal lymph nodes were enlarged and fused. However, after timely localized treatment with the CyberKnife, and continued implementation of NKT cell immunotherapy, the signs of tumor activity were completely suppressed, and since then, there have been no signs of recurrence or metastasis!

Imaging findings: The retroperitoneal lymph nodes are located below the left renal vein, from August 2015 to March 2016 , March 2016 showed slight shrinkage compared to previous images.

Imaging findings: The retroperitoneal lymph nodes are located below the left renal vein. Between August 2016 and August 2017, they significantly shrunk in August 2017, with no significant change in January 2018.

Imaging findings: The small retroperitoneal lymph nodes are located below the left renal vein, with no significant changes between December 2018 and March 2021.

ImagingTableshows: No significant changes were observed in the dorsal segment of the lower lobe of the right lung compared to2019-6-26, suggesting local fibrosis.

Imaging manifestation: A new small nodule is found in the basal segment of the right lower lobe. Close observation and follow-up are recommended.

 

From this phenomenon, we can observe that the onset of NKT cell therapy is relatively slow. In the early stages of treatment, sporadic lesions may appear, but their growth rate is not rapid, their volume is small, and they are easy to intervene in. However, once effective, the therapeutic effect can be maintained for many years, and may even meet the criteria for "cure".

Therefore, it is generally recommended that the duration of cellular immunotherapy be around 2 to 3 years.
Mr. Zhang has been suffering from cancer for nearly 10 years (since 2012), far exceeding the one-year survival period initially predicted by doctors. During the early stages of NKT treatment, although new progress was observed, it was not severe and was immediately controlled.
During the 6 years of treatment, the patient's quality of life has always been very high, and he is in a joyful mood. He is a prolific poet among our friends and relatives, and has already published 5 poetry collections.
From this case, it can be seen that recurrent recurrence and metastasis during the treatment of cancer patients is a common phenomenon, and there is no need for fear or panic.
Whenever new lesions appear, other treatment methods can be used to treat the newly occurring metastatic lesions, reduce tumor burden, and then combine with cellular immunotherapy to "maintain stability". This can achieve the ultimate goal of long-term high-quality life.
